Referral Forms
Making a Referral
If you are a health or social care professional wishing to refer someone to Saint Michael's, please download and complete the appropriate referral form and return it to Saint Michael's Hospice. On receipt of this form we will consider your referral based on need. The speed of admission can be affected by the availability of a service or the nature of the person's illness.
Please download the appropriate referral from, complete it and send it back to Saint Michael's.
From time to time we also receive a small number of referrals for our services from outside our catchment area. In these cases, if the patient is medically suitable and our services are available, Saint Michael's staff will make an out of area request to the appropriate commissioner for funding. If this is granted, we will accept the patient for care.
